Welles Family Student Health Care Center - FAQs

Below are some of the most frequently asked questions that parents have in regards to health information.  If you do not find the answer to your question on this page, please contact the Student Health Care Center by phone at 561.210.2080 or email at infirmary@saintandrews.net.


I did not receive my medical information packet in the mail.  How can I receive this information or download the appropriate health forms?

I did not receive the Blue Immunization Form (DH 680) in the health information packet.  How can I get this form?

I did not receive the Yellow Physical Form (DH 3040) in the health information packet.  How can I get this form?

I received a letter in my health information packet stating that my child's immunization records are incomplete.  What should I do?

My child will be required to take prescription medication while at school.  Do I need to fill out a special permission form?

Will my child be given over-the-counter medication while at school?

My child is in middle school, does he/she still need to fill out the Florida High School Athletic Association (FHSAA) Preparticipation Physical Evaluation Form (EL2)?

My child does not participate in a Saint Andrew's School athletic activity (i.e. equestrian, dance, etc.).  Does he/she still need to complete the Florida High School Athletic Association (FHSAA) Preparticipation Physical Evaluation Form (EL2)?

I did not receive my medical information packet in the mail.  How can I receive this information or download the appropriate health forms?

  • You may call us at 561.210.2080 or email us at infirmary@saintandrews.net to receive the information by mail.  You can also access this information online by visiting the infirmary website.  Once at the infirmary site, please scroll down to view the health requirements specific to your child's grade.

I did not receive the Blue Immunization Form (DH 680) in the health information packet.  How can I get this form?

  • The required Florida Certification of Immunization "Blue Card" form (DH 680) can only be obtained from your physician or the Florida Department of Health.  This form must be completed by a Florida physician or the Department of Health.

I did not receive the Yellow Physical Form (DH 3040) in the health information packet.  How can I get this form?

  • The yellow State of Florida School Entry Health Exam form (DH 3040) can only be obtained from your physician or public health clinic.  This form is only required for new students to Saint Andrew's School and students entering Junior Kindergarten, Kindergarten, and 7th grade.

I received a letter in my health information packet stating that my child's immunization records are incomplete.  What should I do?

  • If your child's immunization records are incomplete, you will receive a letter in your health information packet indicating what is missing on your child's immunization form.  Please have your child's physician update these records and resubmit a new "Blue Card" (obtained from your physician) to the Student Health Care Center indicating completion of all required immunizations.

My child will be required to take prescription medication while at school.  Do I need to fill out a special permission form?

  • Yes.  If your child is required to take any type of prescription medication (i.e. oral, injectable, inhaler, etc.) during school hours, the parent and physician must complete a Physician/Parent Authorization for Student Medication form and submit to the Student Health Care Center.

Will my child be given over-the-counter medication while at school?

  • All parents must complete the Emergency Medical Information Form each year prior to school attendance.  On the back side of this form, parents can indicate whether or not they give permission to the school to administer over-the-counter medication. 

My child is in middle school, does he/she still need to fill out the Florida High School Athletic Association (FHSAA) Preparticipation Physical Evaluation Form (EL2)?

My child does not participate in a Saint Andrew's School athletic activity (i.e. equestrian, dance, etc.).  Does he/she still need to complete the Florida High School Athletic Association (FHSAA) Preparticipation Physical Evaluation Form (EL2)?

  • No.  This form is only required for those middle school and upper school students that participate on a sanctioned Saint Andrew's School athletic team.  However, if he/she is in the upper school he/she should complete an athletic waiver form (obtained from the athletic department) at the beginning of the school year to receive credit for participation.

Does my child need a new physical exam each year?

  • Only students in Junior Kindergarten, Kindergarten, 7th grade, and all students new to Saint Andrew's School (LS, MS, US-Day, US-Resident) are required by the State of Florida Department of Health to have a School Entry Health Exam prior to the new school year.  This exam must be completed by a physician in the United States and an original copy of the physical on the yellow DH 3040 State of Florida School Entry Health Exam form (provided by the physician) must be submitted to the Student Health Care Center prior to attendance.  If your child is in the middle or upper school and will be participating in athletics, he/she will be required to have a sports physical completed on the FHSAA Preparticipation Physical Evaluation Form (EL2) prior to the start of the season.  This form will be handed out to your child by the coach at the start of the season or can be accessed by clicking on the link above.  In addition to the EL2 form, the FHSAA Consent and Release from Liability Certificate (EL3), and the FHSAA Concussion and Heat Related Illnesses Informaton Release Form (EL3CH) must also be completed and returned to the athletic department.

We will be out of town for the summer.  Can I have my child's school entry physical exam performed outside of Florida?

  • School entry physical exams must be performed by a physician in the United States.  A student may have a physical performed in the United States outside of Florida, but must be completed on the yellow DH 3040 State of Florida School Entry Health Exam form obtained from a Florida physician or the Florida Department of Health.

  • The Florida Department of Health requires that school entry health exams be performed by a physician in the United States.  In addition, these evaluations must be completed on the yellow DH 3040 State of Florida School Entry Health Exam form only.  These forms can only be obtained from a Florida physician, Florida Department of Health, or public health clinic.  The charge for this exam by MedExpress is $20.  There will be an additional $10 fee if immunization records need to be transferred to the official "Blue Card". 

My child will be a new resident student attending Saint Andrew's School.  What are the immunization requirements for school attendance?

  • The Florida Department of Health has set specific immunizations requirements for school entry in the state of Florida.  In addition to these state requirements, Saint Andrew's School recommends that all resident students receive the Menactra Meningitis Vaccine.  Parents wishing to decline the meningitis vaccine for their child must sign the waiver on the Mandatory Immunization Form.  Please click here to view all immunization requirements and to access the Mandatory Immunization Form.

Why does Saint Andrew's School recommend that all resident students receive the Menactra Meningitis Vaccine?

  • The Advisory Committe on Immunization Practices (ACIP) currently recommends this vaccine for living in campus residence halls.  Parents wishing to decline the vaccine for their child must sign the waiver form on the Mandatory Immunization Form.  To view information about Meningococcal Meningitis and the Mandatory Immunization Form, please click here.

What are the hours of operation for the Student Health Care Center?

  • The Student Health Care Center is open Monday-Friday from 8:00am-4:00pm during the school year.  During the summer, it is open from 8:00am-3:30pm.  The resident head nurse is on call during the school year during the week and weekends for the residential program.

Where is the Welles Family Student Health Care Center on campus?

  • The Student Health Care Center (#52 on campus map) is located on the southeast corner of the Malnik Family Athletic Center.  Please enter the main upper school entrance on Jog Road and turn right into the parking lot traveling towards the Lower School.  Park and then take the sidewalk that is on the south side of the Malnik Family Athletic Center (in between the Reid Family Pool and Malnik Family Athletic Center).  The Student Health Care Center will be on your left just past the boys' locker room.
