Middle School

• 7th GRADE Students and Students NEW TO SAINT ANDREW'S SCHOOL:

  • Emergency Medical Information Form
  • FHSAA Preparticipation Physical Evaluation Form (EL2)
    • Required for athletic participation.  Submit to athletic department.
  • FHSAA Consent and Release from Liability Certificate (EL3)
    • Required for athletic participation.  Submit to athletic department.
  • FHSAA Concussion and Heat Related Illnesses Information Release Form (EL3CH)
    • Required for athletic participation.  Submit to athletic department.
  • Physician/Parent Authorization for Student Medication
    • This form is only required of those students taking prescription medications during school hours.

If you are participating in a sport this year at Saint Andrew's School, you are required to complete and submit the Florida High School Athletic Association (FHSAA) Preparticipation Physical Evaluation (EL2) Form, Consent and Release from Liability Certificate (EL3), and the FHSAA Concussion and Heat Related Illnesses Information Release Form (EL3CH) prior to participation.  Please submit both forms to Mr. Ashley in the athletic department.

**No student will be able to try-out or participate in athletics without these forms submitted.  Forms must be submitted prior to the start of any try-out/practice.**
