Athletic Forms Required for all Middle School and Upper School Athletes - Updated June 6, 2012

If you are participating in a sport this year at Saint Andrew's School, you are required to complete and submit the Florida High School Athletic Association (FHSAA) Preparticipation Physical Evaluation (EL2) Form, Consent and Release from Liability Certificate (EL3), and the FHSAA Concussion and Heat Related Illnesses Information Release Form (EL3CH) prior to participation.  Please submit both forms to Mr. Ashley in the athletic department.

New students to Saint Andrew's School in Grades 10-12 must also complete and submit the Affidavit of Compliance with FHSAA Policy on Athletic Recruiting.  

Any upper school student that participates in a sport outside of Saint Andrew's School should complete and submit the athletic waiver form below prior to May 1.

**No student will be able to try-out or participate in athletics without these forms submitted.  Forms must be submitted prior to the start of any try-out/practice.**

Upper School Athletic Waiver Form (To be completed by any upper school student that is participating in an activity outside of Saint Andrew's - i.e. dance, equestrian, etc.)
